Becoming a DUTCH Provider is Easy!

To fill out your application, you will need:

Professional name and designation
Company name, address, and NPI number (if applicable)
Contact email and phone number
A copy of your license

Once you complete the Become a Provider form, a member of our onboarding team will connect with you and walk through the steps to begin using DUTCH in your practice. As a provider, you gain access to all of our educational resources and support to help you treat your patients.
